Managing the toxicities of cancer therapy
Chemotherapy-Induced Nausea and Vomiting
Antiemetic prophylaxis is chosen by the emetogenic risk of the chemotherapy regimen.
Prevention beats rescue. The higher the emetogenic potential, the more antiemetic classes you combine up front - uncontrolled CINV predicts worse control on later cycles.
Acute vs delayed
Acute CINV occurs within 24 hours; delayed CINV occurs days 2-5 (especially cisplatin) and needs multi-day coverage. Anticipatory CINV is a conditioned response.
