Heart Failure with Reduced EF (HFrEF)
HFrEF is defined by a left ventricular ejection fraction of 40% or less with signs and symptoms of heart failure.
Chronic HFrEF care slows progression and cuts mortality with guideline-directed medical therapy (GDMT), not just symptom relief. Four drug classes - the 'four pillars' - each independently lower mortality and are started and titrated together.
Stages vs Classes
ACC/AHA Stages A-D describe structural progression (one direction). NYHA Class I-IV describes current symptoms and can move both ways.
