USP Chapter 797 - the rulebook for compounding sterile products

Why Sterile Products Have Their Own Rulebook

The United States Pharmacopeia (USP) and National Formulary published practice standards for compounding sterile preparations after case reports of patient harm and fatality. USP Chapter 797 aims to prevent patient harm from ingredient errors and microbial contamination. It covers personnel responsibilities, training and evaluation, preparation sterility and accuracy verification, classification of microbial contamination risk, and equipment and environmental quality and control.

Sterile compounding is high-stakes: a contaminated injection bypasses every natural barrier and can kill. USP 797 exists because real patients died. It applies to ALL facilities (hospitals, nursing homes, pharmacies) and ALL practitioners (physicians, nurses, technicians), but pharmacists carry it most.
