Septic shock on the ward
65 y M, 24 h post-op, T 39.4°C, BP 88/52, HR 124, lactate 4.2, urine output low.
VTE prophylaxis assessment
72 y F, admitted with pneumonia and reduced mobility. Renal function: CrCl 38 mL/min.
Renal dose adjustment
Patient on vancomycin for MRSA bacteraemia; CrCl drops from 90 to 28 mL/min after AKI.
IV to oral switch
Patient on day 3 of IV ceftriaxone for community-acquired pneumonia. Afebrile, eating/drinking, BP stable.
